Cannabidiol and Autism: Early Signals, Limited Evidence, and Important Safety Questions

Interest in cannabis-based treatments for autism has grown, particularly among families seeking help for anxiety, sleep problems, irritability, aggression, or other co-occurring difficulties. But the evidence does not yet support describing cannabis as an established treatment for autism—or as a way to alter autism’s core characteristics.

The most relevant evidence behind earlier claims comes from a 2019 study published in Frontiers in Pharmacology. Researchers examined reports from parents of 53 children and adolescents in Israel who received a cannabidiol-dominant oil containing CBD and a small amount of THC. The participants were followed for a median of 66 days.

This was not a randomized, placebo-controlled clinical trial. The study had no control group, and the results were based primarily on parents’ reports during telephone interviews. The researchers recorded whether symptoms appeared to improve, remain unchanged, or worsen.

Among the participants who had particular symptoms, parents reported improvement in hyperactivity for 68.4% of 38 children, improvement in self-injury or rage attacks for 67.6% of 34 children, and improvement in sleep problems for 71.4% of 21 children. Anxiety improved in 47.1% of 17 children, while 23.5% were reported to have worsening anxiety. The most frequently reported possible side effects were sleepiness and changes in appetite.

These findings may justify further research, but they cannot show that CBD caused the improvements. Symptoms may change naturally, expectations can influence parent ratings, and other treatments or changes in a child’s environment may also have played a role. The study also did not demonstrate an 80% reduction in repetitive behaviors.

CBD, THC, and the “entourage effect”

CBD is a cannabinoid that is generally not intoxicating, while THC is the cannabinoid primarily responsible for cannabis’s intoxicating effects. Cannabis products can contain widely varying amounts of both compounds, and products sold outside the prescription-drug system may not have consistent potency or purity.

Some researchers and patients believe that combinations of cannabinoids and other plant compounds may work differently from purified CBD alone. This idea is often called the “entourage effect.” It remains a hypothesis rather than a clinically established explanation for autism-related benefits. Evidence from one cannabis formulation should not be generalized to every CBD oil, medical cannabis product, or THC-containing preparation.

There is a biological reason cannabinoids continue to attract research: the endocannabinoid system helps regulate processes including sleep, appetite, memory, mood, and neural signaling. A plausible mechanism, however, is not evidence that a treatment is effective or safe for a particular person.

What later research suggests

Clinical research published since the 2019 observational study has begun to include randomized trials of CBD-rich products, including formulations with small amounts of THC. Reviews of these trials have reported possible improvements in areas such as disruptive behavior, anxiety, and social responsiveness, but they also emphasize the small number of studies, limited sample sizes, differing formulations, and uncertainty about long-term effects. Results should therefore be considered preliminary rather than proof that cannabinoids treat autism.

Research is also increasingly distinguishing between autism’s core traits and co-occurring conditions. A treatment that helps with sleep, severe irritability, pain, or anxiety would not necessarily treat autism itself. Goals should be defined around the individual’s quality of life, communication, comfort, safety, and autonomy—not simply around suppressing harmless autistic behaviors such as stimming.

Epidiolex is not an autism treatment

One might assume that Epidiolex represents a cannabis treatment available for autism. Epidiolex is a prescription formulation of purified CBD. According to the U.S. Food and Drug Administration, it is approved for seizures associated with Lennox-Gastaut syndrome, Dravet syndrome, or tuberous sclerosis complex—not for autism.

That approval does not establish that over-the-counter CBD products or cannabis preparations are safe or effective for autism. The FDA has warned about possible liver injury, sleepiness, and drug interactions with CBD. THC-containing products also raise additional concerns for children and adolescents because THC can affect attention, memory, coordination, mood, and perception.

The American Academy of Child and Adolescent Psychiatry recommends against medical marijuana or isolated cannabinoids for children and adolescents with autism outside appropriate research and emphasizes the lack of reliable information about dosing, product quality, long-term safety, and effectiveness.

What about probiotics?

Probiotics may be useful for some gastrointestinal problems, but they should not be presented as a proven treatment for autism-related behavior. The National Center for Complementary and Integrative Health says current clinical evidence does not support using probiotics to modify behavior in people with autism. Families considering a probiotic should discuss the specific gastrointestinal symptoms, product, and possible risks with a clinician rather than relying on claims made for branded “autism” formulations.

For now, cannabinoid treatment for autism remains an area of investigation. Anyone considering CBD or a THC-containing product—especially for a child or teenager—should consult a qualified healthcare professional, review potential interactions with current medicines, and avoid replacing established supports without medical guidance. Larger, well-controlled studies are still needed to determine which symptoms, if any, may respond, what formulations and doses are appropriate, and what risks may emerge with long-term use.
