D-Limonene Reduced Some Acute Anxiety and Paranoia Effects of THC in a Small Human Study

A small controlled human study found that vaporized D-limonene—the citrus-scented terpene found in cannabis and many citrus fruits—may lessen some of the anxiety and paranoia that can accompany THC. The findings suggest that cannabis compounds can interact in specific ways, but they do not show that limonene treats anxiety disorders or that consumers can reliably prevent unwanted effects by choosing a limonene-rich product.

Published in Drug and Alcohol Dependence in 2024, the study was led by researchers at Johns Hopkins and supported in part by the National Institute on Drug Abuse. Twenty healthy adults who used cannabis intermittently completed a series of double-blind laboratory sessions. Participants inhaled vaporized THC alone, D-limonene alone, THC combined with D-limonene, or distilled-water placebo.

The researchers tested 15- and 30-milligram doses of THC alongside 1 or 5 milligrams of D-limonene. A smaller group of 12 participants also completed a session combining 30 milligrams of THC with 15 milligrams of D-limonene. Investigators assessed participants’ subjective drug effects, mood, cognitive and psychomotor performance, vital signs, and blood concentrations of THC and limonene for up to six hours.

The clearest effect appeared at the highest limonene dose. Compared with 30 milligrams of THC alone, the combination of 30 milligrams of THC and 15 milligrams of D-limonene significantly reduced participants’ ratings of feeling “anxious/nervous” and “paranoid.” Anxiety-related ratings generally declined as the limonene dose increased, but the lower-dose comparisons did not produce the same statistically significant result.

D-limonene did not substantially change the other acute effects of THC. Measures of cognitive performance, physiological responses, overall drug effects, and other subjective experiences were broadly similar when limonene was added. When inhaled without THC, D-limonene produced no clear effects that differed from placebo, and it did not appear to alter the way the body processed THC.

These findings provide limited human evidence for one aspect of the cannabis “entourage effect”—the idea that non-THC compounds can modify the effects of THC. However, the study tested isolated chemicals delivered by vaporization, not commercially available cannabis strains, edibles, or typical consumer products. The highest-dose combination was administered only to 12 people and was always tested in the final session, an important design limitation acknowledged by the researchers.

The results therefore should not be interpreted as proof that D-limonene can prevent cannabis-related panic or paranoia in everyday use. The doses, delivery method, THC strength, individual tolerance, and expectations in a laboratory experiment may differ substantially from real-world cannabis use. The study also examined short-term reactions in healthy adults rather than long-term treatment of an anxiety condition.

Even with those limitations, the findings could inform the development of THC-based medicines with a wider therapeutic window. Future research will need to determine whether the effect occurs with oral formulations, persists in larger and more diverse groups, and applies to combinations of naturally occurring cannabis compounds rather than isolated vaporized ingredients.
