High-Dose CBD Intensified the Effects of Oral THC in a Small Human Trial
A 2024 clinical trial found that a large oral dose of cannabidiol (CBD) did not soften the effects of tetrahydrocannabinol (THC). Instead, it increased several of THC’s subjective and measurable effects in healthy volunteers.
The study published in Clinical Pharmacology & Therapeutics tested 37 healthy adults in a randomized, double-blind, placebo-controlled, five-way crossover trial. On separate visits, participants received a placebo, 9 milligrams of oral THC alone, or the same THC dose combined with 10, 30, or 450 milligrams of CBD.
The researchers had expected CBD to reduce THC-related adverse effects, an idea that has influenced both public perceptions of CBD and some theories about cannabis formulations. The results did not support that expectation at the doses tested. When participants took 450 milligrams of CBD with THC, they reported stronger effects and showed greater changes in psychomotor performance, cognition and autonomic measures than when they took THC alone. Ratings of feeling high increased by about 60.5 percent compared with THC alone.
The lower CBD doses—10 and 30 milligrams—did not significantly intensify THC’s subjective or performance-related effects. However, the researchers found evidence of a pharmacokinetic interaction at 30 and 450 milligrams: CBD increased exposure to THC and its metabolites in the bloodstream. The findings are consistent with CBD slowing the enzymes involved in oral THC metabolism, particularly cytochrome P450 pathways.
That mechanism is important because the study examined swallowed cannabinoids, not smoked or vaporized cannabis. Oral THC passes through the digestive system and liver before reaching the bloodstream, so CBD’s effect on metabolism may differ by route of administration. The results therefore should not be treated as proof that every CBD-containing cannabis product will intensify THC in the same way.
The trial also found no evidence that adding CBD improved THC’s analgesic effects. In fact, the combination did not provide a clear pain-relief advantage in the experimental pain tests used in the study. This is a significant limitation on any claim that CBD makes THC more tolerable or more useful for people with chronic pain: the participants were healthy volunteers, received single doses, and were not patients undergoing treatment for a pain condition.
The findings add to earlier human research showing that CBD can alter THC exposure after oral consumption. A randomized clinical trial published in JAMA Network Open likewise reported stronger drug effects and higher THC exposure when a high dose of CBD was taken with oral THC. Together, these studies suggest that CBD’s reputation as a universal counterweight to THC is too simplistic.
They also highlight why dose, product composition and method of use matter. Over-the-counter products may contain different amounts of CBD and THC than their labels indicate, and CBD can interact with other medicines. The U.S. Food and Drug Administration’s CBD guidance warns about potential drug interactions and liver-related risks, particularly with medicinal doses of CBD.
Other cannabis compounds may behave differently. For example, a separate 2024 study of 20 adults found that vaporized D-limonene reduced some THC-related anxiety and paranoia without changing other measured THC effects. That result is preliminary and involved inhaled THC and a terpene rather than oral THC and CBD, but it illustrates why the so-called “entourage effect” cannot be assumed to operate uniformly across cannabinoids, terpenes, doses or delivery methods.
The practical takeaway is narrower than the original study’s most dramatic interpretation: a single high oral dose of CBD enhanced the effects of 9 milligrams of THC in a small group of healthy adults and did not improve experimental pain responses. More research is needed to determine how these interactions vary with lower doses, repeated use, inhaled products, prescription medicines and people who use cannabinoids therapeutically.
