CBD and Sleep: Why the Dose May Matter

CBD is widely marketed as a non-intoxicating way to support better sleep, but the research does not support a simple “more is better” or “CBD is sedating” conclusion. Its effects appear to vary with dose, formulation, the person taking it and the reason they are having trouble sleeping.

One of the earliest human studies, published in 1981, involved just 15 people with insomnia. Participants received oral doses of 40, 80 or 160 milligrams of cannabidiol, a placebo or the sedative drug nitrazepam. The 160-milligram CBD dose was associated with more reported sleep than placebo, while the lower doses did not show the same effect. Because the study was small and relied heavily on subjective reports, it provides an early signal—not a validated dosing guide. Read the original 1981 clinical study.

Later research produced a more complicated picture. A 2008 study in rats found that CBD increased wakefulness when administered directly to brain regions involved in the sleep-wake cycle. Animal findings cannot be translated directly into human treatment recommendations, but they help explain why CBD may not make every user feel sleepy.

Human evidence has also been inconsistent. In a 2018 randomized, double-blind crossover study, healthy volunteers received a single 300-milligram dose of CBD or placebo. Researchers found no acute changes in the participants’ sleep-wake cycle. The result does not prove that CBD can never affect sleep, but it does challenge the idea that a large dose will reliably act as a sleep aid for healthy adults. See the randomized sleep study.

A frequently cited 2019 report examined CBD use in a psychiatric clinic. The retrospective case series included 72 adults who were being treated for anxiety or sleep complaints alongside their usual care. Sleep scores improved during the first month for 48 participants, or 66.7 percent, but the improvements fluctuated over time. The study did not include a placebo control, CBD dosing was not standardized, and many participants did not remain in follow-up. As a result, it cannot establish that CBD caused the improvement. These limitations are consistent with a 2014 systematic review of human cannabinoid-and-sleep studies, which described the findings as mixed and cautioned that differences in dose, delivery method, study design and participants made firm conclusions difficult. Much of the broader sleep literature also concerns cannabis products containing THC or people whose sleep was affected by pain or another medical condition, rather than CBD alone in otherwise healthy adults.

For people new to CBD, the practical takeaway is that there is no established over-the-counter CBD dose proven to treat insomnia. A product that helps one person may do little for another—or may cause drowsiness, gastrointestinal symptoms or changes in alertness. Commercial products can also contain substantially more or less CBD than their labels indicate. CBD products sold for sleep have not been approved by the U.S. Food and Drug Administration for that purpose.

CBD can interact with prescription medicines and has been associated with liver injury in some settings. The FDA has approved purified CBD only for certain rare seizure disorders, where it is used under medical supervision. Anyone considering CBD—particularly people who take regular medication, have liver disease, are pregnant or breastfeeding, or are treating persistent insomnia—should discuss it with a health professional rather than relying on trial and error.
