Cannabinoid Bioavailability Depends on Route, Formulation and Timing

There is no single “best” way to take a cannabinoid. Inhaled products generally produce the fastest and highest blood concentrations, oral products last longer but are absorbed unpredictably, and conventional topical products are intended mainly for localized effects rather than delivery into the bloodstream.

Bioavailability is the proportion of an administered substance that reaches the systemic circulation and can produce an active effect. For cannabinoids such as delta-9-tetrahydrocannabinol (THC) and cannabidiol (CBD), bioavailability depends not only on the intake route but also on the formulation, dose, inhalation technique, food intake, and individual differences in metabolism.

A 2018 review of cannabinoid pharmacokinetics and pharmacodynamics found that route of administration strongly influences how quickly cannabinoids enter the blood, the concentrations they reach, and how long their effects may persist. The percentage estimates below are broad ranges—primarily reported for THC—and should not be treated as precise predictions for every cannabis product.

Inhalation: fastest onset and relatively high systemic exposure

Smoking or vaporizing cannabis allows cannabinoids to pass through the lungs and enter the bloodstream rapidly. Peak blood concentrations are typically reached within minutes, making inhalation the quickest route when a rapid onset is important.

Reviews commonly estimate inhaled THC bioavailability at roughly 10% to 35%. The wide range reflects differences in puff volume, inhalation duration, breath-holding, device design, particle size, the amount of active ingredient delivered, and how much of the dose is lost or swallowed. Inhaled CBD has also shown relatively high systemic availability in human studies, but estimates vary substantially between products and experiments.

The speed of inhalation comes with trade-offs. Effects can be more intense and shorter-lived than those from oral products, and smoking exposes the lungs to combustion products. Vaporization may avoid combustion but is not risk-free, particularly when products are poorly regulated or contaminated. Inhaled THC can also impair attention, reaction time, and driving ability.

Oral administration: slower, longer-lasting and highly variable

Edibles, capsules, oils swallowed as food or medicine, and other oral products pass through the digestive tract before reaching the liver. This “first-pass” metabolism converts some THC into 11-hydroxy-THC, an active metabolite that can contribute to the distinctive and sometimes delayed effects of edibles.

Oral THC bioavailability is generally lower and more variable than inhaled THC, often estimated at approximately 4% to 12% in pharmacokinetic reviews. Absorption may take 30 minutes or longer, with peak effects commonly occurring much later—sometimes after two hours or more. The delayed onset can make it easier to take additional doses before the first dose has fully taken effect.

Oral products may provide a longer period of symptom relief, but their effects are influenced by the product’s oil or capsule formulation, the individual’s digestive function, and whether the dose is taken with food. A systematic review of human CBD studies found that diet, dose, and formulation were major determinants of CBD exposure; taking CBD with food generally produced higher blood concentrations than taking it while fasting. The review also found that results varied considerably across studies.

Because oral absorption is difficult to predict, people using an edible or other oral product should follow the product’s labeled serving size, allow sufficient time for effects to develop, and avoid combining it with alcohol or other sedating substances.

Topical and transdermal products are not the same

Topical creams, lotions, balms, and gels are applied to the skin. Conventional topical products are generally designed to act at or near the application site. Cannabinoids may interact with local skin tissues, but there is limited evidence that typical retail topicals deliver reliable, clinically meaningful amounts into the bloodstream or central nervous system. For that reason, it is not accurate to assume that every topical product produces systemic effects—or to assign all topical products a single bioavailability range such as 0% to 4%.

Transdermal delivery is different. A transdermal formulation is specifically engineered to move an active ingredient through the skin and into systemic circulation, often using penetration enhancers or specialized delivery systems. Research reviews describe transdermal cannabinoid delivery as promising, but much of the evidence remains experimental, formulation-specific, or based on limited human data. The 2022 review of transdermal therapeutic cannabinoids discusses these opportunities and the substantial challenges created by cannabinoids’ poor water solubility and highly lipophilic nature.

Other routes under study

Oromucosal and sublingual products—such as sprays, lozenges, and dissolving films—are absorbed partly through the tissues of the mouth and may reduce, but not necessarily eliminate, first-pass metabolism. Their absorption depends on how long the product remains in contact with the mouth and how much is eventually swallowed.

Nasal, rectal, and other delivery systems have also been studied. These routes may help researchers develop products with faster onset, more consistent dosing, or improved bioavailability, but they are less established in everyday cannabis use than inhaled and oral products.

Which route is most effective?

If “effective” means fastest onset and relatively efficient delivery into the bloodstream, inhalation generally has the advantage. If the goal is longer-lasting effects, oral administration may be more suitable, although its delayed onset and variable absorption require caution. If the goal is localized skin or muscle application, a topical product may be appropriate, but it should not automatically be expected to produce whole-body effects.

Bioavailability is only one consideration. The appropriate route also depends on the cannabinoid, product formulation, desired onset and duration, medical history, other medications, and the risks associated with inhalation or psychoactive effects. The U.S. Food and Drug Administration’s information on CBD products notes that CBD can interact with other medications and may cause adverse effects, including liver injury in some circumstances.

In short, inhalation tends to deliver cannabinoids quickly, oral products tend to last longer but vary more from person to person, and topical products are primarily intended for local use unless they are specifically formulated for transdermal delivery. Understanding those differences can help set realistic expectations—but it cannot replace product-specific dosing guidance or advice from a qualified healthcare professional.
