Cannabis Concentrates for Medical Use: Potency, Options, and Safety Considerations
Cannabis concentrates can deliver far more THC in a small amount than traditional cannabis flower. That high potency may appeal to some adults using cannabis under a state medical-cannabis program, but it also increases the chance of unwanted effects, impairment, and difficulty judging an appropriate dose.
Cannabis concentrates are products made by separating cannabinoids and other compounds from the cannabis plant. Common forms include wax, budder, shatter, hash oil, live resin, rosin, distillate, and kief. “Extract” and “concentrate” are often used interchangeably, although product definitions can vary by state and manufacturer.
How potent are concentrates?
Concentrates commonly contain substantially more THC than cannabis flower. Reviews of products available in regulated markets have reported typical concentrate potency in the roughly 50%–70% range, with some products labeled above 90%. The percentage on a package describes concentration, not the amount a patient should use in one session. A very small amount can contain a large THC dose.
Product labels may list THC as a percentage, milligrams per gram, or total milligrams in a cartridge or container. Because labeling rules and testing practices differ, purchase products only from licensed retailers where they are legal, and ask the dispensary about laboratory testing, serving size, ingredients, and contaminants.
How concentrates are made
Some concentrates use solvents such as butane, ethanol, or carbon dioxide during extraction. Others are produced with methods commonly described as solventless, including dry sifting, ice-water separation, or heat and pressure. These terms describe how the product was made; they do not by themselves indicate that a product is safer or more appropriate for medical use.
Never attempt to make a solvent-based concentrate at home. Solvents such as butane can ignite or explode, causing serious injury. Products from unregulated sources may also contain pesticides, residual solvents, heavy metals, or other contaminants.
Ways people use concentrates
- Vaporizing or dabbing: A concentrate is heated and inhaled through a specialized device. This can produce effects quickly, but the high THC dose may be difficult for an inexperienced or sensitive user to control. Cannabis oils and concentrates used for vaping or dabbing may also contain additives or contaminants, and vaping has been linked to lung injury.
- Vape cartridges: These products are designed for specific devices and may be easier to use than a dab rig. Follow the product instructions and avoid modifying cartridges or using products from informal sellers.
- Oral use: Some oils and concentrates are formulated for oral or sublingual use, while others may be incorporated into food. Swallowed cannabis can take 30 minutes to two hours to produce noticeable effects, and the effects may last longer. Taking more before the first dose has taken effect is a common cause of overconsumption.
Adding an unmeasured amount of concentrate to a joint, pre-roll, or homemade food can make the dose difficult to estimate. For medical use, a measured product with clear labeling is generally easier to track than a product portioned by eye.
Potential medical benefits—and important limits
Research on cannabis and cannabinoids suggests modest short-term benefits for some people with chronic pain and multiple-sclerosis-related spasticity, but the evidence is not the same as proof that high-THC concentrates will treat those conditions. As the National Center for Complementary and Integrative Health explains, research on many other conditions remains limited or is still in its early stages.
Concentrates should not be presented as an established treatment for Alzheimer’s disease, intractable pain, or epilepsy. In the United States, the Food and Drug Administration has not approved cannabis itself to treat any disease. It has approved the prescription CBD medicine Epidiolex for seizures associated with Lennox-Gastaut syndrome, Dravet syndrome, and tuberous sclerosis complex. Epidiolex is a standardized prescription medication and is not equivalent to a THC-rich cannabis concentrate purchased through a dispensary.
State medical-cannabis laws differ widely. A condition that qualifies a patient for a medical-cannabis program in one state may not qualify in another, and some jurisdictions impose potency limits on particular products. Legal availability also does not establish that a product is safe or effective for a specific diagnosis.
Risks patients should discuss with a clinician
THC can impair memory, attention, coordination, judgment, and reaction time. High-concentration products may produce intense anxiety, panic, paranoia, confusion, hallucinations, rapid heart rate, severe nausea, or vomiting. Frequent use of highly concentrated THC products is associated with more severe cannabis use disorder symptoms, although research is still developing and does not establish that concentrates alone cause every reported health problem.
Smoking can expose the lungs and cardiovascular system to harmful combustion products. Vaping and dabbing avoid burning plant material but are not risk-free, and the long-term effects of inhaling concentrated cannabis aerosols remain incompletely understood. Do not drive, operate machinery, or combine cannabis with alcohol, sedatives, opioids, or other intoxicating substances.
Keep concentrates and infused foods in their original, child-resistant packaging and locked away from children and pets. Do not use THC products during pregnancy or breastfeeding; the FDA advises against cannabis, THC, and CBD in any form during these periods.
A cautious approach
Before considering a concentrate, review your goals, current medications, medical history, prior cannabis experience, and risk of substance use disorder with a licensed healthcare professional. If a clinician supports trying cannabis, ask whether a lower-THC product or a noninhaled, measured formulation would be more appropriate. Use the smallest labeled amount, avoid taking additional doses too quickly, and record the product, dose, timing, benefits, and side effects.
