Cannabis Hangover Evidence Is Limited, But It Does Not Rule Out Impairment

A 2023 systematic review found limited evidence that cannabis use impairs cognitive or safety-sensitive performance more than eight hours later. But the findings do not show that people are safe to drive or perform hazardous work after using cannabis—and they do not establish that drug-testing policies are unjustified.

Published in Cannabis and Cannabinoid Research, the review examined 20 interventional studies involving 458 participants and 345 performance tests conducted more than eight hours after exposure to delta-9-tetrahydrocannabinol, or THC, the primary intoxicating component of cannabis.

Most of the assessments did not find a statistically significant “next-day” effect. Twelve tests—3.5% of the total—reported poorer performance after THC, while three suggested improved performance. Results from 121 tests were classified as unclear because the studies did not provide enough information to determine whether THC had affected performance.

The evidence was also uneven. The five studies containing the 12 negative findings did not use randomized, double-blind, placebo-controlled designs, and all were published more than 18 years before the review. By contrast, several studies reporting no next-day impairment did use stronger controlled designs. The authors nevertheless emphasized that many studies had methodological limitations and often failed to report effect sizes clearly.

The review addressed impairment occurring after the acute effects of cannabis—not whether cannabis is safe immediately after consumption. Experimental research has consistently found that THC can impair driving-related skills and cognitive functions in the hours following use. The National Highway Traffic Safety Administration advises that people should not drive after using impairing substances, noting that cannabis can affect motor skills, lane tracking and multitasking.

The distinction matters because a positive drug test is not the same as proof of current impairment. Federal workplace-testing guidance from the Substance Abuse and Mental Health Services Administration states that drug-test results do not determine an employee’s fitness for duty. Depending on the specimen, testing may identify THC itself or a THC metabolite, which can remain detectable after the period of intoxication has ended.

That gap creates a policy challenge for employers and regulators, particularly in safety-sensitive industries. A test designed to detect prior cannabis exposure cannot by itself answer the more important question: whether a person is impaired at the time they are driving or working. The NHTSA’s review of drug-impaired-driving laws similarly notes that THC and its metabolites are not reliable standalone indicators of impairment and that evidence remains insufficient to determine whether zero-tolerance or per se laws improve safety outcomes for cannabis.

The review’s conclusions are therefore narrower than claims that cannabis has no residual effects. It found that most available tests did not detect measurable impairment beyond eight hours, but the studies were small, varied in quality and focused largely on healthy participants rather than medical-cannabis patients or regular users. The researchers called for better-designed studies, including trials examining different doses, routes of administration, cannabis products and medicinal use.

For now, the findings support a more precise discussion of cannabis policy rather than a blanket conclusion that all post-use restrictions should be removed. Rules aimed at preventing on-duty impairment should be informed by performance and behavioral evidence, while policymakers should recognize that detecting THC exposure is not equivalent to measuring a person’s ability to drive or safely perform a job.
